Root Cause / Diagnostic:
High headphone monitoring volume or open-back earphone acoustic leakage bleeds directly into a nearby sensitive condenser microphone. The recycled audio creates hollow phase cancellation, hollow room tone, or high-pitched feedback loops.

Actionable Fix:
1. Switch talent to closed-back, well-sealed in-ear monitors (IEMs) fitted with expanding memory foam ear tips.
2. Lower headphone amplifier monitor volume to the minimum comfortable listening level (around 65–70 dB SPL).
3. Mute talent speech and crank mic gain on monitors; confirm absolute silence on the microphone channel during headphone playback.
